About this role
Care Manager RN provides support for care management and care coordination activities and collaborates with a multidisciplinary team to coordinate integrated member care.
Key Responsibilities
- Completes comprehensive assessments of members per regulated timelines and determines who may qualify for care management based on clinical judgment, changes in member health or psychosocial wellness and triggers identified in assessments
- Develops and implements care coordination plan in collaboration with member, caregiver, physician and/or other appropriate health care professionals and member support network to address member needs and goals
- Conducts telephonic, face-to-face or home visits as required
- Performs ongoing monitoring of care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly
- Maintains ongoing member caseload for regular outreach and management
Technical Overview
Uses EMR and HIPAA compliance; coordinates with ICT and leverages Microsoft Office for documentation and reporting.

The ideal candidate is an RN with care management experience who can coordinate care across disciplines, travel as needed, and communicate effectively with patients and providers.
Must-Have Skills
At least 2 years experience in health carepreferably in care managementor experience in a medical and/or behavioral health settingRegistered Nurse (RN). License active and unrestrictedValid and unrestricted driver's licenseUnderstanding of the electronic medical record (EMR) and HIPAADemonstrated knowledge of community resourcesAbility to operate proactively and demonstrate detail-oriented workAbility to work within a variety of settings and adjust style as needed - working with diverse populationsAbility to work independentlywith minimal supervision and self-motivationResponsiveness in all forms of communicationand ability to remain calm in high-pressure situationsAbility to develop and maintain professional relationshipsExcellent time-management and prioritization skillsand ability to focus on multiple projects simultaneously and adapt to changeExcellent problem-solvingand critical-thinking skillsStrong verbal and written communicationMicrosoft Office suite/applicable software program proficiency
