Clinical Liaison- CM/ RN

Clinical Liaison (CM/RN) coordinates rehab referrals, performs pre-admission screening, and provides education to patients and families. The role guides patients through rehabilitation DRGs and the discharge rehabilitation care continuum while collaborating across disciplines to follow evidenced based standards.

• Coordinate referrals from physicians, case management, and health care facilities
• Perform pre-admission screening for the rehab program
• Provide significant patient education related to illness and rehabilitation recovery
• Guide patients with rehabilitation DRGs through the discharge rehabilitation care continuum
• Develop and implement a referral program and collaborate with all disciplines

This role centers on clinical workflow coordination and rehab program operations: referral coordination, pre-admission screening, patient education, and discharge planning tied to rehabilitation DRGs and evidenced based standards. It uses healthcare and productivity tools including Allscripts, Microsoft Office, and ACCESS, plus standard office equipment like printers and fax.

The ideal candidate has 2+ years of clinical experience in a health care environment, preferably in a hospital, and possesses a current Texas State License in Clinical Specialty. They are strong in pre-admission screening and patient education for rehab recovery, with experience coordinating referrals and collaborating across disciplines; they are also BLS-qualified with AHA Healthcare Provider BLS within 60 days of hire.
