Provide telehealth primary and urgent care as a CareBridge Nurse Practitioner for patients receiving home and community-based services. You will develop clinical plans of care, conduct history/physical assessments and use diagnostics to set treatment plans, educate patients and families, and maintain compliant medical records.

• Provides primary and urgent health care via telephone and tele video modalities to home and community-based patients
• Develops and implements clinical plans of care for adult patients with chronic and complex conditions
• Gathers history and physical exam and uses diagnostics to develop treatment plans
• Identifies and closes gaps in care and supports physical and psychosocial needs
• Maintains patient medical records and coordinates with other physicians and medical providers
