About this role
Serve as a CareBridge Advance Practice Provider (Nurse Practitioner) delivering primary and urgent care through telephone and tele video to patients in home and community-based settings. Develops clinical plans of care and treatment plans, coordinates with physicians and providers, educates patients and families, and maintains compliant medical documentation.
Key Responsibilities
- Provides primary and urgent health care via telephone and tele video modalities to home and community-based patients
- Develops and implements clinical plans of care for adult patients with chronic and complex conditions
- Gathers history and physical exam and uses diagnostics to develop treatment plans
- Identifies and closes gaps in care and meets physical/psychosocial needs
- Maintains patient medical records and coordinates with other physicians and providers
Technical Overview
This is a telehealth-focused clinical role rather than a technology implementation position. The work centers on history/physical and diagnostics-driven care planning, documentation consistent with state regulations and company policy, and coordination across an interdisciplinary care team.
