About this role
This Nurse Practitioner role (CareBridge) provides primary and urgent care for adult patients via telephone and tele video. The provider develops clinical plans of care and treatment plans, educates patients and families, closes gaps in care, and maintains documentation consistent with state regulations and company standards.
Key Responsibilities
- Provide primary and urgent care via telephone and tele video modalities
- Develop and implement clinical plans of care and treatment plans for adult chronic conditions
- Gather history and physical exam and use diagnostics as needed
- Educate patients and families on medication, adherence, and crisis prevention
- Maintain patient medical records and coordinate care with interdisciplinary teams per state regulations
