DaVita IKC is hiring a Nurse Practitioner to provide holistic, integrated care for patients with complex kidney conditions in Plainfield, IN with some travel. The role focuses on completing Comprehensive Health Evaluations (CHEs), managing CKD/ESKD, reducing hospitalizations, and coordinating care without nights, weekends, or on-call.
Key Responsibilities
- Complete Comprehensive Health Evaluations (CHEs)
- Manage CKD and ESKD patients
- Perform medication management and diagnostic interpretation in collaboration with nephrologists
- Lead care coordination to align with medical treatment plans
- Deliver clinical leadership and support disease progression prevention
Technical Overview
Clinical care role with telehealth (occasional work from home) and travel across dialysis clinics and nephrology practices. Utilizes intermediate office computer skills (MS Word, Excel, Outlook) and may involve EHR documentation (Cerner or similar).
Ideal Candidate
The ideal candidate is a Nurse Practitioner with 2+ years of clinical experience managing complex, chronic kidney patients (CKD/ESKD). They have current, unrestricted NP licensure and DEA eligibility, strong autonomous clinical judgment, and experience with care coordination and Comprehensive Health Evaluations (CHEs).
