About this role
Provide RN case management for DaVita Integrated Kidney Care (DaVita IKC) for high-risk patients to reduce hospital readmissions. Begin work from home using email/dashboard review and then conduct field visits to dialysis centers and nephrology offices for comprehensive health evaluations and care coordination.
Key Responsibilities
- Manage a caseload of high-risk patients (~100+)
- Travel to dialysis centers or nephrology offices for visits during treatment/appointments
- Conduct Comprehensive Health Evaluations (CHEs) head-to-toe assessments
- Teach patients about comorbidities (diabetes, heart failure) and reconcile medications
- Complete heavy documentation for risk adjustment and care plans and coordinate care
Technical Overview
The role combines remote monitoring with structured clinical assessment and documentation (risk adjustment and care plans). It requires strong medication reconciliation, patient education, and coordination with a Nurse Practitioner and Care Coordinator.
Ideal Candidate
The ideal candidate is a senior clinical RN Case Manager with at least 5 years of clinical nursing experience and an active Registered Nurse (RN) license. They can manage a high-risk patient caseload, perform Comprehensive Health Evaluations (CHEs), conduct medication reconciliation, and complete the documentation required for risk adjustment and care plans while coordinating care with a Nurse Practitioner and Care Coordinator.
