About this role
Serve as an RN Case Manager for DaVita Integrated Kidney Care (DaVita IKC), managing a caseload of high-risk patients and coordinating their care to reduce hospital readmissions. Work begins from home via email/dashboard review, with field visits to dialysis centers and nephrology offices to perform comprehensive health evaluations.
Key Responsibilities
- Manage a caseload of high-risk patients (~100+)
- Travel to dialysis centers or nephrology offices for visits during treatment/appointments
- Conduct Comprehensive Health Evaluations (CHEs) head-to-toe assessments
- Teach patients about comorbidities (diabetes, heart failure) and reconcile medications
- Complete heavy documentation for risk adjustment and care plans and coordinate care
Technical Overview
Role blends remote monitoring and heavy clinical documentation (risk adjustment and care plans) with in-person field visits. It requires strong clinical assessment, medication reconciliation, and structured care coordination with a Nurse Practitioner and Care Coordinator.
Ideal Candidate
The ideal candidate is a senior clinical RN Case Manager with at least 5 years of clinical nursing experience and an active Registered Nurse (RN) license in the state of practice. They can manage a high-risk patient caseload, perform Comprehensive Health Evaluations (CHEs) head-to-toe assessments, complete heavy documentation for risk adjustment and care plans, and coordinate care with a Nurse Practitioner and Care Coordinator.
