About this role
Step-Down Navigator manages the workflow of the step-down process from inpatient to PHP/IOP, engages patients within 24-36 hours of admission, and coordinates with Social Services, UR, and Outpatient Departments to verify transition feasibility.
Key Responsibilities
- Meet with eligible patients within 24-36 hours
- Confirm transition feasibility with involved departments
- Document step-down information in MS4 and Midas
- Review reports to identify trends
- Report weekly findings
